- Birth = Hep B
- 1-2 Months = Hep B
- 2 Months = Rotavirus, DTaP, Hib, PCV13, Inactivated Poliovirus
- 4 Months = Rotavirus, DTaP, Hib, PCV13, Inactivated Poliovirus
- 6 Months = (Rotavius & Hib see notes cdc chart), DTaP, PCV13
- 6-18 Months = Hep B, Inactivated Poliovirus, Influenza annually
- 12-15 Months = Hib, PCV13, MMR, Varicella, Hep A (2 dose series see notes cdc chart)
- 15-18 Months = DTaP
- 2 & 3 Years= Influenza annually
- 4-6 Years = DTaP, Inactivated Polio, Influenza annually, MMR, Varicella
- 7-10 Years = Influenza annually
- 11-12 Years = Influenza annually, Tdap, (HPV see notes cdc chart), Meningococcal
- 13 – 15 Years= Influenza annually
